5/15/2023 EnchantaVaughn Approved (AM) Upon arrival, client was observed entering class in a uptight mood, as evidenced by him cringing up his face and frowning. Client presented a negative attitude toward his teacher and counselor due to being upset that he had to deal with the consequences of his actions. Client was standoffish and distant, ignoring debriefing and refusing to communicate his feelings. The client continues to be very cautious about expressing his emotions and was encouraged to do this at his own pace. The client has to be reminded to communicate positive feedback and not to let his ego get in his way of learning and receiving help. Client was not acceptive of redirection and responded aggressively by saying “I don’t want to talk to you”. (PM) Client chose to follow behind the good behaviors when lining up for lunch. Client symptoms of ADHD have stabilized due to avoiding distractions. Client transitions to lunch smoothly and safely. Client was given permission to sit with counselor during lunch. It has been reported that client had a rough day. Client was observed out of his seat and unsupervised in the hallway. Client refused to stay in his seat and asked his counselor to call his mom so he could go home. Client was inquired why he wanted to go home. Client explained he didn’t want to be in school. Client was frustrated at having to be in class so long and attempted to skip class for the rest of the day. Client hates being redirected by other teachers and becomes very defiant, displaying disrespectful attitude and ignoring teacher prompts. Client was given positive feedback from his counselor to make better choices and to find something he likes about school and focus on that. Client continued to complain and avoid instruction. Client was not in a happy mood, as evidenced by his negative comments and aggressive facial expressions. QMHP told client that he has a choice to stay home or come to school and get an education. Client was encouraged to make the most of his day since he was already here and try to cheer up. Client was reminded of his objectives ad how many days there are left to end the year on a good note. Client finally calmed down and went to class. (EOD) Client continues to build new relationships in school as a way of making school fun for him. Client demonstrates positive social interactions in his class and in specials. Client was able to finish the day without aggressive or defiant behaviors. Client prepares for announcements. Client is supported for his cooperation during dismissal and leaving school safely. Client did not meet objectives.
